will make a chart for you with your doctor's orders, our
notes, and other medical information. We use this chart to
record your treatments and medical condition. You can request
to see this information at any time with an appropriate
request. Only people with legal rights to see your chart or
others you tell us about in writing can see it.

respect. We will keep your treatment and personal needs

of respect, you will be addressed as Mr., Mrs., Miss, or
Ms. unless you ask to be called by another name. |

consent form must be signed before any information is

Problem
Solving Procedure
We will protect your
rights at our agency. If you feel our staff has not done a
good job or has, in any way, denied you your rights, please contact
the Director Of Nursing or Clinical Manager at our offices

not need to fear discrimination or revenge.
