Sometimes
things happen that result in transfer, referral, or discharge
of your care from Dimensional HomeCare Services. Some of these
situations include, but are not limited to:

a
change in your condition — you may need more, less, or
no care |

we
believe you or our staff are not safe in your home |
|
you
do not do what your doctor tells you to do |
We
will tell you before you are to be
discharged (except in case of
emergency). We will provide you with information regarding
other agencies and assist you in changing your care if
transfer is required. If this happens, we will let the agency
you choose know what is needed for your continued care. If
discharged, you will be given information which will assist
you in taking care of yourself. |
